Hello I hope someone understands what im trying to say, back in the old days of guild was you where able to see what kind of debuff the enemy has on them when you click and target a enemy, such as if i use conjure phantasm on a enemy i click i was able to see it under their name. is there any way to do this or anything can be done to see what kind of stuff the enemy has on them or my team mates?

You know if a target is hexed if they have a pink down-pointing arrow at the end of the health bar. What you're talking about has never existed (unless it was in beta and removed for proper release). If the bar is pink you can determine that they are suffering from some kind of health degeneration, but other than that, there is no easy way to check.

Yeah I know what you're talking about, but Guild Wars has never had such a thing (I don't even think that the down arrows indicating hexes and conditions have been around the whole time).

Unter the monster's name you see which skill they are activating ... did you cast conjure phantasm on a mesmer monster with conjure phantasm? It's the only way I can imagine that...
